What you need to do
Thank you for your interest in helping with this ministry.
All volunteers need to complete the Criminal Background Check (use the request form below, or from https://epatch.state.pa.us/Home.jsp.) and Child Abuse History Clearance forms. In addition, the Volunteer Application and Waiver forms must be completed and submitted for consideration.

All completed and approved forms can be sent to us at X-Stream Youth Ministry, P.O. Box 336, Mt. Wolf, PA 17347-0336. Upon review, we will contact you to schedule a time for an interview.
